Job Description
The ng Transform Diabetes Care RN is responsible for engaging telephonically with members, to present actionable steps to address diabetes gaps in care, evaluate their medical needs and facilitate the member’s overall wellness by utilizing their expertise as a CDE/CDCES. The nurse will Utilizes advanced clinical judgment and health related critical thinking skills to deliver this information.
External candidates must reside within 60 miles & near to the Downers Grove, IL office for training & meetings.
Schedule is Monday-Friday 8am-8pm CST. At least 2 evening shifts per week.
No weekends and no holidays!
Fundamental Components
Through the use of clinical tools and information/data review, applies clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address complex health and social indicators which impact care planning and resolution of member issues.The Transform Diabetes Care Pod RN is responsible for conducting telephonic outreach to for the purpose of delivering care gap specific interventions to members with diabetes in order to facilitate care gap closure. The RN is responsible to refer to and collaborate care with member/authorized representative and PCP when needed. The RN will also assist with community resource identification/education and referral as needed. The RN is responsible for documenting accurately and timely in the electronic health record. This position requires the RN to use critical thinking and be able to problem solve any issues related to assigned membership.
Background Experience
3 years of clinical nursing experience required
Previous coaching experience preferred.
LICENSE & CERTIFICATION REQUIRED
RN with current unrestricted Illinois state licensure required, multiple state licensure a plus.
CDE/CDCES required (Certified Diabetic Educator or Certified Diabetic Care & Education Specialist)
